The Telemonitor SNS 24 app is designed for users integrated into telemonitoring programs of the National Health Service (SNS).
The application allows users to register and consult health information within the context of these programs, including biometric parameter measurements obtained through external devices, when applicable, and self-assessments entered by the user.
The registered data can be consulted by healthcare professionals responsible for monitoring the user within the respective telemonitoring program.
The application functions as a tool for registering and sharing health information. It does not perform measurements on its own, does not make diagnoses, does not replace medical advice, and should not be used for emergency situations.
