Diaverum d.CARE patient app

d.CARE is an app developed and intended for Diaverum’s renal patients, which allows them to:
• Report their daily well-being to their clinic
• Visualize their current and historical treatment values
• Keep track of their latest and historical lab results
• Remind themselves about their current prescribed medication and what it is for.

As a patient you can optionally opt in to sharing your health data with your clinic.
This includes:
*Steps data: To track your daily activity.
*Sleep data: To track your overall sleep.
